Give Women A Lift

The Teal Tea Foundation is proud to support the "Give Women A Lift" Program. "Give Women A Lift" offers transportation to women with Ovarian or other Gynecological Cancers who are facing financial hardship. The program has helped hundreds of women undergoing radiation or chemotherapy treatment to travel between home and their doctor's office, hospital or cancer clinic.


Most recipients of Give Women A Lift are women who do not have a ride or are unable to drive themselves to treatments and appointments. The program provides reimbursement for taxi and care service costs. Requests are made by case workers, social workers or patient care coordinators; and funds are distributed through the case worker or directly to a qualified transportation company.


Please consider making a donation to Give Women A Lift. The program has recently expanded its reach to New Jersey, New York and Pennsylvania and the need is greater than ever. 

 

Program Summary


The Teal Tea Foundation, a 501(c)(3) not-for profit organization, officially established the “Give Women a Lift” program (GWAL) on January 1, 2012 funded by a grant received from the Zonta International organization to provide the cost of public or private round-trip transportation for women receiving treatment for ovarian cancer and/or other gynecological cancers, including cervical, uterine, vaginal and vulvar and living within the area of New Jersey and southeastern Pennsylvania (including the counties of Berks, Bucks, Chester, Delaware, Lancaster, Lehigh, Philadelphia, Montgomery and Northampton). GWAL will also cover transportation costs for medical office follow up visits related to the treatment of ovarian cancer and/or for any other gynecological cancer, as outlined above.

 

Any woman that is receiving treatment for ovarian and/or gynecological cancer and is in financial hardship as determined by their medical care provider is eligible to apply. Applicants can apply to GWAL program through her doctor’s office or treatment facility’s Social Services Department.


Transportation funding covers round trip expenses for qualified medical office follow up visits related to cancer treatment up to $500.00 per person per calendar year.

 

Program Guidelines for Patients

 

1. To be eligible you must be a patient with a diagnosis of ovarian cancer and/or any other form of gynecological cancer and are currently in treatment at an accredited hospital, medical center, clinic or not-for-profit health care facility, and travel to a medical office for treatment or follow up visits related to cancer treatments.

2. Contact the Social Service Representative at your treatment facility and provide them with the Program Guidelines for Social Services Representatives which explains the “Give Women a Lift” program and the accompanying Give Women a Lift Request for Transportation Funding to complete. The Request for Transportation Funding may be downloaded from the Teal Tea website at www.tealtea.org.

3. Provide the Social Service Representative with requested information to determine your charity care status.

4. Advise the Social Service Representative if an ADA Reasonable Accommodation related to the transportation is needed prior to date of first grant covered service.

5. Advise the provider of transportation services if an ADA Reasonable Accommodation related to the patient’s transport is needed.

6. Agree to cooperate with the treatment facility and/or transportation providers by submitting the current voucher on date of transport; be ready for pickup on your appointment date; observe the safety rules and regulations for the mode of transportation, as determined by the transportation provider.

7. Notify the Social Service Representative of any change in your status that might have an impact on your transportation grant funding.


Program Guidelines for Social Service Representatives


1.Give Women a Lift will cover transportation costs for medical office visits related to the treatment of ovarian cancer and/or for any other gynecological cancer for patients living in the defined program area (New Jersey and southeastern Pennsylvania including the counties of Berks, Bucks, Chester, Delaware, Lancaster, Lehigh, Philadelphia, Montgomery and Northampton).  The patient must be receiving treatment at an accredited hospital, medical center, clinic, or not-for-profit health care facility; or are traveling to a medical office for treatment or follow up visits related to the cancer treatment. Patients must also demonstrate a need for financial assistance.

2. Transportation funding covers round trip expenses for qualified medical office follow up visits related to cancer treatment up to $500.00 per person per calendar year.

3. Complete the Request for Transportation Funding on behalf of the patient and submits the form to the GWAL Coordinator.

4. Gift cards will be mailed or reimbursement will be made directly to the public or private transportation provider or the hospital within 30 days of receipt of the completed request.

5. Arrange for ADA Reasonable Accommodation relative to the patient’s transportation, as needed.
